Calvary Silver Circle collects personal information from you and from your carers, your representatives and the service which has referred you to Calvary Silver Circle, with your consent. The personal information we collect may include your personal details, information about your health and services you may use or require, information about your family and carers and information about your living arrangements and lifestyle.
Calvary Silver Circle will use your personal information to plan and co-ordinate your services. We will use it also to invoice you or the agency purchasing your services. As part of managing our business we are required to be audited for quality assurance and accreditation from time to time and your personal information may be used in these audits.
We may use your personal information to inform you of other services and products which we believe may be of interest or relevance to you. You may contact your Service Coordinator or our Calvary Silver Circle Privacy Officer if you do not wish us to use your information for this purpose.
Calvary Silver Circle may disclose some personal information to other service providers, agents and contractors from time to time in order to provide services to meet your needs.
Calvary Silver Circle will not provide your personal information to any other party or use this information for any other purpose than that for which it was originally intended, without your consent, unless we have a legal obligation to do so or in an emergency situation where you may be endangered and not able to provide consent.
Calvary Silver Circle uses a variety of security measures to safeguard your personal information and keep it secure from misuse, loss or unauthorised use, including restricting access to our offices and files, and secure electronic databases. Once service has ceased, we are required to save data for 7 years and have arranged for hard copy files to be archived in secure premises, before being confidentially destroyed.
We endeavour to ensure the personal information we hold is accurate, complete and up-to-date. However, the accuracy of the information we hold depends largely on the information you provide. Please let us know if you are aware of any errors in your personal information and keep us up-to-date with any changes to your personal information.
You have a right to access your personal information, subject to some exceptions allowed by law, and to request a change to your information if you consider it is inaccurate. You may contact our Privacy Officer at privacy@silvercircle.com.au if you would like to access your personal information.
In some circumstances, where the information is in the past and has been archived, Calvary Silver Circle may charge a fee for searching for and providing access to your information.
